A HIPAA waiver form is an instrument used for legal purposes where it allows a person's health information to be disclosed or be used by a third party and which the information is held by another entity in trust. This is usually for medical facilities, doctors, schools, or any other person where they are in possession of the health information of an individual and where the said individual authorizes the former to release the information to a third person. The information here is personal medical information which, by law, prohibits the possessors of such information from disclosing it without the prior authorization given.
