A patch test consent form is a consent acquisition form used for getting the consent of patients or clients for conducting a patch test prior to the actual procedure to be made. This is for the purpose of determining whether the client is allergic to any of the ingredients that are to be used with him/her to the procedure. A patch test is scheduled and conducted by a physician which then can be assessed for a period of 48 hours to see whether an allergic reaction has occurred on the skin area where the patch test is made. A patch test is a standard diagnostic test for allergic contact dermatitis. This is done to ensure the safety of clients reducing or preventing possible risks and dangers from the procedure that may cause injury or death.
