A New York State Traveler Health Form is a personal information collection tool used by the New York State for getting to know their travelers, who they are, and the places they have been to or their intended places to visit. This form is primarily designed to ensure conducting basic health checks on travelers to ensure or mitigate the spread of COVID-19 over the state. Thus, through the information gathered, the state may conduct preliminary processes to ensure that the person is not infected by the dreaded virus, such as conducting quarantine procedures for travelers who might have been to places with high rates of infection.
